Welcome to the 2022-2023 Tier IV Co-ed U8, U10, U12, & U14

with Maine Evolution!

This registration session will allow you to sign consents, complete contact information and submit payment.  To complete registration through our secure site, please have your Visa, MasterCard, Discover or PayPal account information available.  Following the completion of this registration, you will see a charge on your account from Maine Evolution.

This registration form is for players who will be playing for the following Tier IV levels: 

8U: 2022-2023 fee $625

10U: 2022-2023 fee $900 ($800 for registrations prior to September 1st)

12U Player: 2022-2023 fee $900  ($800 for registrations prior to September 1st)

12U Goalie: 2022-2023 fee $450 ($400 for registrations prior to September 1st)

14U Player: 2022-2023 fee $900   ($800 for registrations prior to September 1st)

14U Goalie: 2022-2023 fee $450  ($400 for registrations prior to September 1st)

10U, 12U, and 14U are required to purchase jerseys. (8U do not need a jersey)

Please download Code of Conduct form and provide to your players team administrator at the first practice. 

OPEN SKATE (to prep for evaluations /no charge) 

Sunday October 23rd from 3-4pm (for 8U, 10U, and 12U age groups)

EVALUATION DATES:

8U Evaluation: Wednesday October 26th 5-6pm

10U/12U Evaluations: Saturday October 29th 12:50-1:50pm

Each Player must register first with USA Hockey and then return to this Registration Session to signup for the 2022-2023 Season.

• Step 1:  CLICK HERE to go to USA Hockey Registration and register your player(s). Be sure to PRINT and SAVE their confirmation code(s). Remember to register EACH of your players with USA Hockey. Each player must have their own unique USA Hockey confirmation number.
• Step 2:  Return to this registration and proceed. Later in this registration session you will need to enter the USA Hockey Confirmation Code that you received in Step 1.

PAYMENT PLAN:

**PAY ONLINE IN FULL: 

• Submit total fees online today upon conclusion of this registration session via Visa, MasterCard, Discover or PayPal account.

** ONLINE PAYMENT PLAN:  

• Fees are paid in 3 installments with the first installment to be collected online today upon conclusion of this registration session via Visa, MasterCard or Discover account. The remaining balance will be automatically debited from the SAME ACCOUNT 
  • If you wish to use a different account to pay the balance, please log in to your SportsEngine account BEFORE the first day of payment to update your account information.
  • PayPal is not an accepted form of payment with this payment option.
